Case Management ServiceCase managementCase management is about the timely co-ordination of services to meet an individual's specific needs in a cost-effective manner and can be provided as required at any or all stages of a persons recovery and reintegration to ensure a co-ordinated approach. The aim is to provide a service, which is cost-effective and responsive to the needs of people with acquired brain injury, and to the satisfaction of their families, solicitors, insurers, statutory and voluntary organisations and others who may be involved. I try to limit my case management workload to those who live no more than an hour's drive from Derby. Expert assessment reportsI am happy to take instructions for case management 'expert' work without geographic constraints. Reports are prepared for the Courts and reflect the systematic and comprehensive approach of the service. Clients are assessed and evaluated so that the full range of their needs can be identified and recommendations made. Following preliminary enquiries and/or instructions and the agreement of terms of business I arrange to see the client - defendant or claimant - at their own home and usually, though not always, with a close family member or friend. Information is gathered as appropriate in order to prepare a detailed report which usually contains a brief summary of the accident details, a full summary of the problems which the client is experiencing as a result of the accident or injury and details of how these impact on his or her life together with any changes in lifestyle that have resulted. Following a summary of the main issues in the case, the client's long term care/support needs are identified and recommendations made as to how these needs will best be met in the future. Reports are proofed and edited before being sent out. Most reports are completed within a month as long the necessary information is available. Reports which are required at very short notice can also be supplied though inevitably these usually take a different and shorter form depending on specific requirements. Preparing and running workshops and seminarsTraining is individually planned and devised with the needs of specific individuals or organisations in mind and can include such topics as an introduction to brain injury, brain injury awareness, training for care workers and volunteers, and communicating with people who have a brain injury. Fees & termsMy fee is charged at a rate of £100 an hour for all work undertaken. Travel time is charged at £42 per hour pro rata and mileage is charged at 50p per mile. Expenses will be claimed as incurred. Usual terms are that payment should be made within 28 days. |
